Job summary

Qualifications

• High school diploma or equivalent is required.
• Six months of related work experience is required.
• Must possess valid Georgia driver’s license with no moving violations.
• CPR training and Georgia Class III Driver’s license are required by the fourth month of employment.
• Equivalent combination of education and experience.

Responsibilities

• Assist clients with activities of daily living and home management.
• Provide transportation to and from doctors or facilities.
• Document visits and services in accordance with procedures.
• Provide outreach and follow-up services.
• Coordinate with case managers and inform them of crises.
• Maintain up-to-date client records weekly, monthly, and quarterly.
• Monitor medically-related situations in coordination with physicians and families.
• Perform housekeeping tasks such as cleaning, laundry, and meal preparation.
