In this role you will provide personal care, assist with daily living activities, monitor clients for changes, document care accurately, and maintain positive relationships with clients, families, and the care team.

Qualifications

• 1 year of experience as a Home Health Aide or related healthcare role.
• Completion of a state-approved Home Health Aide training program.
• Ability to lift, push, and pull up to 50 pounds, and stand for extended periods.
• Excellent communication and interpersonal skills.
• Compassionate, patient demeanor and a patient-centered approach.

Responsibilities

• Provide personal care services, including bathing, dressing, and grooming.
• Assist with daily living activities, such as medication management, meal preparation, and light housekeeping.
• Monitor and report changes in clients' condition to the care team.
• Develop and maintain positive relationships with clients, families, and caregivers.
• Maintain accurate and detailed records of client care services and communications.
