Job summary

Live Well Home Care is seeking caregivers to provide in-home support to clients in their own homes and communities. Responsibilities include assisting with daily living activities, meal planning, housekeeping, transportation, and companionship according to care plans.

Caregivers ensure safety, document notes, and collaborate with the branch team to support clients’ wellbeing and independence.

Responsibilities

• Assist with activities of daily living and personal care such as ambulation, bathing, dressing, exercise, feeding, hair care, medication reminders, mouth care, nail care, positioning, shaving, skin care, toileting, transferring.
• Ensure client’s safety and security by supervising the home environment.
• Teach/perform meal planning and preparation, routine housekeeping (beds, dusting, vacuuming, floors, kitchen and bathroom, laundry).
• Provide companionship through social interaction, conversation, emotional reassurance and stimulating activities.
• Provides respite care for families per care plans.
• Perform/assist with essential shopping/errands, may include handling client’s money under observation of Branch Manager.
• Assist clients with following a written special diet plan and reinforcement of diet maintenance as identified on the care plan.
• Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan.
• Assist clients with written communication by writing or typing correspondence.
• Participate with Client Care Team by providing input and suggestions.
• Ensure service is delivered per applicable policies, procedures, and practices.
• Monitor supplies and resources.
• Follow the written care plan.
• Carry out duties as assigned by the Branch Manager.
• Observe clients and environments and report unsafe conditions to Branch Manager.
