Qualifications

Responsibilities

• Support mobility and safe transfers as needed.
• Prepare meals and help with eating.
• Perform light housekeeping related to client needs.
• Provide medication reminders and support adherence to care plans.
• Monitor, observe, and document changes in client condition.
• Communicate clearly with clients, families, and care coordinators.
• Protect client privacy and follow safety procedures.
