Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• Minimum of one year's nursing experience preferred.

Responsibilities

• Oversee primary caseload of Hospice patients providing skilled nursing care.
• Conduct initial and comprehensive nursing assessments of patients.
• Implement and reevaluate the care plan based on patient needs.
• Educate and support patients and families regarding nursing care.
• Supervise Hospice Aides and Homemakers.
