Qualifications

• Registered nurse with current licensure to practice in Arizona.
• Minimum of two years’ experience, with at least one in public health, home care, or hospice.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Perform comprehensive patient assessments to determine hospice needs.
• Develop and implement patient care plans with interdisciplinary teams.
• Ensure continuity and safety of care for patients in homes.
