Job summary

Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a full-time Hospice Nurse Practitioner to join our dedicated team. You’ll provide expert clinical support, symptom management, and compassionate guidance to patients and families in end-of-life care.

Collaborating with physicians, nurses, social workers, and chaplains, you’ll help ensure comfort, dignity, and quality of life. Flexible Monday–Friday schedule with a comprehensive benefits package is offered.

Qualifications

• Current Nurse Practitioner license (CRNP) in the state of Pennsylvania.
• Hospice or palliative care experience preferred.
• Excellent communication and collaboration skills.

Responsibilities

• Provide hospice patient assessments, symptom management, and clinical direction in collaboration with the hospice medical director.
• Work closely with the interdisciplinary team to create and adjust individualized plans of care.
• Offer education and emotional support to patients and their families.
• Participate in team meetings and ongoing quality care initiatives.

Are you a compassionate, skilled clinician who feels called to make a difference in the lives of patients and families facing end-of-life care? Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a full time Hospice Nurse Practitioner to join our dedicated team.

As a valued member of our hospice care team, you’ll provide expert clinical support, symptom management, and compassionate guidance to patients and their loved ones. You’ll collaborate with physicians, nurses, social workers, and chaplains to ensure comfort, dignity, and quality of life are always at the heart of what we do.

What You’ll Do as a Hospice Nurse Practitioner:
