Job summary
Colorado PACE is seeking an experienced LPN to provide home health care in alignment with our mission of compassionate, patient-centered care. You will collaborate with the IDT/IDG and RN Case Manager to ensure the Plan of Care is followed and patient outcomes are optimized.
The role requires a valid Colorado LPN license, CPR certification, and a minimum of one year in hospice or related fields. Weekend availability may be required; competitive hourly pay and strong benefits support our team’s
Qualifications
- Minimum Education: Graduate of an accredited school of practical nursing.
- One year of hospice, palliative care, home health, or related clinical experience.
- Current Colorado LPN license and CPR certification.
- Valid Colorado driver’s license and automobile insurance as required by state law.
Responsibilities
- Provide patient home health care in alignment with scope and standards.
- Participate in Interdisciplinary Team/Group (IDT/IDG) meetings.
- Coordinate with RN Case Manager and assigned RN to follow the Plan of Care.
- Monitor patient's physical, emotional, and spiritual needs.
- Educate patients and families on hospice and home health treatments.
