Job summary

Four Seasons offers comprehensive CNA roles providing day-to-day personal care to individuals with chronic or life-limiting conditions. CNAs must meet licensure requirements and follow the written plan of care.

Under the Community Clinical Team Leader, you will participate in hospice coordination, collect data for assessments, contribute to the care plan, educate patients and families, and maintain safety and infection control standards.

Qualifications

• Education: High School Diploma or GED is required.
• Experience: 0–1 year of clinical experience; no more than a 24-month lapse after CNA training.
• Licensure: Valid NC CNA certificate and valid driver's license with proof of insurance.

Responsibilities

• Practice within license scope and follow plan of care as delegated.
• Participates in hospice care coordination per professional standards.
• Collects data for interdisciplinary assessments and communicates observations.
• Contributes to plan of care development and evaluation.
• Assists with counseling and education for patients and families.
• Complies with Medicare Conditions of Participation and infection control.
• Provides hands-on personal care and maintains safety practices.
• Works to achieve performance goals and aligns with Four Seasons values.
