Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to join our compassionate team serving patients at home and in care settings. You will perform personal care, review care plans, and assist with lifting, mobility, nutrition, and daily activities under the supervision of a Case Manager RN.

Qualifications

• CNA license is required
• 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice is required
• Current CPR certification is preferred
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license is required
• Valid Auto Insurance is required

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
