Job summary

Interim HealthCare seeks a compassionate Social Worker to join our home health team, providing personalized care in patients' homes. You will assess social and emotional needs, collaborate with the interdisciplinary team, and ensure resources are available to improve health and quality of life.

This Per-Diem role offers potential to transition to part-time or full-time, with travel to patient homes in our service area and HIPAA-compliant documentation responsibilities.

Qualifications

• Degree in Social Work from an accredited school of social work.
• Current and valid state licensure as a Social Worker (LCSW or LMSW or equivalent).
• Experience in home health care or healthcare settings preferred.

Responsibilities

• Conduct psychosocial assessments in patients' homes to identify social and environmental factors.
• Develop and implement individualized care plans addressing psychosocial needs.
• Provide counseling and emotional support to patients and families.
• Assist navigating healthcare systems and accessing resources.
• Collaborate with physicians, nurses and other professionals for integrated care.
• Coordinate communication among patients, family, and care team.
• Provide discharge planning and continuity of care after hospital stays.
• Educate on disease management and community resources.
• Advocate for services and document progress per policies and HIPAA.
