CenterWell Home Health in Texas seeks a Home Health Registered Nurse to provide one-on-one skilled nursing care in patients' homes, administer medications, perform wound care, and assess conditions. You will develop individualized care plans with physicians and other clinicians and monitor progress.

Responsibilities include coordinating with healthcare professionals, educating patients and families, maintaining records, and supervising Home Health Aides.

Qualificações

• Experience in Med-surg, ICU, ER, or acute care settings.
• Active RN license and CPR certification.
• Ability to collaborate with a healthcare team.
• Valid driver's license and reliable transportation.

Responsabilidades

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
