Job summary

CenterWell Home Health is seeking a dedicated Home Health Registered Nurse to deliver one-on-one care in patients' homes, coordinating with physicians and clinicians to support recovery and independence. As part of the team, you will develop and implement individualized care plans, monitor patient conditions, educate patients and families on disease management, and supervise Home Health Aides.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wounds, and assessments.
• Develop and implement individualized care plans with physicians and other professionals.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management, medications, and treatments.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient’s physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
