Job summary

CenterWell Home Health is looking for a dedicated Home Health Registered Nurse to deliver one-on-one nursing care in patients’ homes, under the supervision of a Clinical Manager. You will participate in care planning with physicians and clinicians and educate families on disease management and treatment options.

The role requires RN licensure, CPR, and prior acute care experience, plus a valid driver’s license and reliable transportation to visit patients’ homes.

Qualifications

• Experience in med-surg, ICU, ER or acute care settings.
• Current and unrestricted RN licensure.
• CPR certification at time of hire.
• Experience collaborating with a healthcare team.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes, administer medications, wound care, and patient assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records and coordinate with healthcare professionals.
• Report patient progress to physician and Clinical Manager.
• Oversee Home Health Aide documentation.
