Job summary

BAYADA Home Health Care is seeking a Registered Nurse to join our home health visit team in South Carolina. This weekend, per-visit RN role serves adult and geriatric clients in Lancaster, York, and Chester counties, delivering skilled nursing and rehabilitative care in the patients' homes.

You will perform head-to-toe assessments, wound care, post-acute care, medication reconciliation, and safety monitoring, while coordinating with a multi-disciplinary team.

Qualifications

• Current nursing license in good standing in the state of South Carolina.
• Graduation from an accredited nursing program.

Responsibilities

• Identify subtle changes in patient condition through head-to-toe assessments.
• Provide skilled nursing and rehabilitative care in adult and geriatric home settings (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
