BAYADA Home Health Care is seeking a full-time Registered Nurse (RN) to join our home health visit team serving Wilkes and Yadkin counties in North Carolina. You will provide skilled nursing and rehabilitative care in clients' homes, collaborating with a multidisciplinary team to keep patients at home with compassionate, high-quality care.

Qualified candidates have an active North Carolina RN license, BSN or higher, and at least 1 year of adult/geriatric clinical experience in various settings.

Qualifikationen

• Current nursing license in good standing in North Carolina.
• Graduation from an accredited nursing program (RN).
• At least 1 year of medical experience with adult/geriatric population in relevant settings.

Aufgaben

• Perform head-to-toe assessments and identify changes in patient condition.
• Care for adult/geriatric clients in home and community settings (acute and post-acute).
• Triage and independently manage urgent clinical situations at home.
• Reconcile medications, provide patient teaching and safety monitoring.
• Conduct wound assessment, care, and documentation.
• Document care at point of care using electronic systems.
