Job summary

Qualifications

• Graduation from an approved certification/program reflecting ability to perform job as described.
• Holds a valid/unencumbered license to practice in the state where services are provided.
• Professional experience as required per state regulations to perform the job.
• Successful completion of screening/competency testing with a proficiency score of 80% or greater.
• Reliable transportation.
• Provide copy of current TB results or Chest x-ray.
• Provide copy of current CPR certification as per agency policy.
• Be flexible, organized and able to exercise independent judgment.

Responsibilities

• Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
• Performs nursing care per physician orders and plan of treatment during each shift.
• Documentation of care in medical records and nursing notes.
• Notify Director/Supervisor of new problems or changes.
• Observe aseptic technique as required and administer meds as per care plan.
• Collaborate with physicians and other health professionals as needed.
