Qualifications

• Current and unrestricted Registered Nurse licensure.
• Minimum of one year of nursing experience preferred; strong medical-surgical, ICU, ER, or acute experience desirable.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient; provide necessary follow-up.
• Coordinate care with a multidisciplinary team.
• Implement patient care plans with patients and families.
