Job summary

Embrace is seeking a Registered Nurse to administer skilled nursing care in patients' homes and coordinate with a multidisciplinary team. The role requires independent work with a focus on patient comfort, safety, and family communication.

Responsibilities include developing and implementing Plans of Care, documenting patient progress, and educating patients and families. Requires an active State RN license, CPR, valid driver’s license, and ability to travel locally.

Qualifications

• Advanced written and verbal communication skills
• Organizational skills
• Ability to supervise in accordance with Agency policies and applicable laws
• Advanced math skills related to patient care
• Ability to respond to inquiries or complaints from regulatory agencies or the community

Responsibilities

• Administers skilled nursing care in patients' homes as prescribed by the physician and coordinated with a multi-disciplinary team.
• Documents patient status, interventions, and education accurately and timely.
• Develops and implements written Plan of Care aligned with physician orders.
• Communicates effectively with patients, families, physicians, and staff.
• Participates in on-call rotation and visits as scheduled.

Education

Graduate of an accredited school of nursing
