Job summary

Humana's CenterWell Home Health is seeking a Medical Social Worker to evaluate and plan personalized social and emotional care for home health patients. You will supervise a Social Work Assistant, coordinate with the care team, and support discharge planning while ensuring compliance with company and regulatory standards.

You will assess patient needs, connect families with community resources, and document care activities, contributing to patient recovery and independence in the home setting.

Qualifications

• Master's or doctoral degree from a school of social work accredited by the CSWE.
• Social Work licensure in the state of practice; if required by state law or regulation.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Proof of current CPR certification
• Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
• Knowledge of and the ability to assist with discharge planning needs, and to obtain community resources.
• Excellent oral and written communication and interpersonal skills.

Responsibilities

• Assesses the patient's social and emotional state as it relates to illness or injury, needs for care and response to treatment.
• Assesses relationships of medical/nursing needs in the home, resources, and community supports.
• Provides actions to obtain community resources to aid recovery.
• Instructs patients and families in coping with social and emotional responses; conducts ongoing assessments.
• Assists the physician and health team in understanding social/emotional factors; participates in care plan development.
• Monitors changes in patient condition and contributes to discharge planning.
• Participates in interdisciplinary care team care coordination; acts as a resource to others.
• Supervises the Social Work Assistant to ensure compliant provision of services.
• Maintains and submits documentation including case conferences and visit reports.
