Qualifications

• LPN/LPT/LVN degree with current unrestricted registration and license in IL.
• Valid driver’s license with clean driving record.
• CPR certification and nursing experience preferred, especially in home health.

Responsibilities

• Provide 1:1 home health care following the plan of care while upholding patient dignity.
• Administer medications, feedings, and monitoring as ordered, with documented outcomes.
• Coordinate supplies and communication with physicians, case managers, and family.
