Qualifications

• 1 year of home health experience preferred.
• Strong nursing knowledge and patient safety standards.

Responsibilities

• Chart observations of patient condition at visits.
• Administer physician-prescribed medications.
• Dress or redress wounds and monitor healing progress.
• Educate patients and families on home health procedures.
• Coordinate with physicians, PTs, OT and other care team members.
• Recommend tools or devices to improve quality of life.
