Qualifications

• Valid LPN license in good standing.
• Proven experience as a licensed practical nurse, preferably in home health.
• Strong knowledge of nursing care methods and procedures.
• Knowledge of health and safety guidelines and willingness to follow them.
• Excellent teamwork and communication abilities.
• Compassionate and responsible with strong multitasking skills.

Responsibilities

• Chart observations of the patient’s condition at each visit.
• Administer physician-prescribed medication.
• Dress or redress wounds and monitor healing progress.
• Provide education to patients and families on home health care procedures.
• Coordinate with physicians and therapists on the patient care plan.
