You will support the RN, assist with procedures, educate families, and monitor patient conditions. A valid VA license and reliable car are required, with weekend on-call rotation as needed.

Qualifications

• Must be a Licensed Practical Nurse with a current license to practice nursing in Virginia.
• At least one year of home health experience preferred.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car for work hours.

Responsibilities

• Supports the Registered Nurse in providing care based on each patient’s plan; assists with procedures as directed by the RN and physician; completes visit notes after each patient interaction; monitors patient condition and participates in on-call rotation.
