Job summary

Tennessee Quality Care is hiring a Full-Time Licensed Practical Nurse (LPN) to serve patients across Lewis, Perry, and Hickman Counties. You will provide direct nursing care in patients' homes and implement the plan of care under supervision.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Minimum of 3 years nursing experience; home health or medical/surgical preferred.
• Current Tennessee LPN or LVN license.
• CPR certification must be valid.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provides direct patient care as defined by the state Nurse Practice Act.
• Implements plan of care initiated by the RN.
• Documents care in a timely and accurate manner per the plan of care.
• Educates patient and family on self-care techniques relevant to diagnosis and plan.
• Coordinates home health services and reports needs to RN or Clinical Supervisor.
• Uses equipment safely and adheres to aseptic technique.
• Participates in growth and development activities.
• Performs other duties as assigned by the RN.
