Qualifications
- Licensed Practical Nurse (LPN) with current Michigan license.
- Minimum 1 year nursing experience, preferably in home health care.
- Strong clinical and assessment skills.
Responsibilities
- Provide routine nursing care and health assessments in patients' homes.
- Administer medications and injections as prescribed.
- Monitor patient conditions and report changes to the team.
- Assist with wound care and catheter care as needed.
- Maintain accurate patient records and documentation.
- Educate patients and families on health maintenance and disease prevention.
- Collaborate with healthcare professionals for coordinated care.
