Job summary

Interim HealthCare Inc. is seeking a Home Health Licensed Practical Nurse (LPN) to provide compassionate, one-on-one in-home patient care in Collinsville and surrounding areas.

The role requires an active VA LPN license, graduation from an accredited practical nursing program, and at least 1 year of LPN experience (home health preferred). CPR and TB skin test are required, with flexible part-time scheduling offered.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in Virginia.
• Minimum of 1 year of LPN experience (home health preferred) or completion of office preceptor program.
• CPR certification and TB skin test required.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based nursing care to patients under supervision of an RN.
• Monitor patient condition, obtain vital signs, and perform routine assessments.
• Document patient progress and report changes to RN supervisor.
• Assist with implementing individualized patient care plans.
• Educate patients and families on medications, treatment plans, self-care, and home safety.
• Coordinate patient care with physicians, RNs, CNAs, aides, and interdisciplinary team.
