Bethesda Group is seeking a compassionate CNA for in-home care in Colorado Springs, CO. The role covers ADLs and IADLs, plus restorative and supportive care, in collaboration with an interdisciplinary team. You will document care in EHR and monitor health status under a care plan.

The position requires a CO CNA license, high school diploma, and at least 1 year of relevant experience; benefits and a comprehensive package are provided.

Formación

• High School Diploma or equivalent.
• Must be licensed as a Certified Nursing Assistant (CNA) in Colorado.
• Experience with elderly or disabled populations preferred.

Responsabilidades

• Travels to participants' homes to assist with ADLs per care plan.
• Assists with restorative and supportive care including ambulation and transfers.
• Monitors health status and observes medications usage.
• Communicates concerns to Home Care Coordinator promptly.
• Documents services in Electronic Health Record (EHR).
• Participates in care planning with IDT.
