Job summary

Community Horizons is seeking caregivers in Wisconsin for day and PM shifts in client homes. Two shifts are available: Day shift 8:00 AM–2:00 PM or 8:00 AM–3:00 PM, and PM shift 4:00 PM–10:00 PM, Monday through Friday and every other weekend.

Wage ranges from $17.00 to $20.00 per hour depending on health care experience. Responsibilities include meal prep, medication administration, transportation, ROM exercises, transfers, dressing, bathing, housekeeping, and standby transfer support.

Qualifications

• Must be at least 18 years old.
• Valid Driver’s License and a reliable vehicle.
• Home health experience preferred but not required.

Responsibilities

• Meal preparation and daily living assistance.
• Medication administration as directed.
• Transportation to appointments and community activities (vehicle provided).
• Assist with range of motion exercises, transfers, dressing, bathing, and toileting.
• Housekeeping and general home care duties.
• Provide standby assistance with transfers (no lift required).
