Job summary

Penn Medicine is seeking a Home Health Aide to implement a Plan of Care, assist patients with daily living activities, and support family members in providing care at home. The role covers home visits within a designated service area and requires timely patient documentation.

Responsibilities include direct patient care, mobility assistance, and maintaining accurate records in line with Medicare/JCAHO standards. Excellent teamwork with RN Case Managers and Scheduling Coordinators is essential.

Qualifications

• HS Diploma or GED required.
• Certification as a Nursing Assistant (PA) or Home Care Aide (NJ) depending on location is required or student nursing assistant with 3.0 GPA in nursing program.
• Minimum 1 year experience as Nursing Assistant or Home Health Aide; or 6 months with nursing student status in BSN program.
• Driver's license required.

Responsibilities

• Direct patient care including bathing, oral hygiene, skincare, and assistance with activities of daily living.
• Assist with mobility, transfers, and bed/chair positioning.
• Assist with pre-poured medications and provide respite care as assigned.
• Accurately document care provided in a timely manner per Medicare/JCAHO standards.
• Communicate changes in patient condition to the nurse and supervisor.
• Follow Plan of Care and complete hours as scheduled.
