Job summary

Modani Care in Vidalia, GA is seeking a compassionate Home Health Caregiver to join our team and help clients maintain independence and quality of life in their own homes.

You will provide personal care and assist with daily activities, medications, and mobility while coordinating with the care team to monitor health changes. A reliable driver’s license is preferred and you’ll work in a supportive, client-centered setting.

Qualifications

• Ability to lift, transfer, and move clients as needed.

• Reliable transportation and a valid driver's license.

Responsibilities

• Provide personal care assistance, including bathing, dressing, grooming, and toileting.
• Assist with daily living activities, such as meal preparation, medication management, and transportation.
• Perform light housekeeping and laundry tasks.
• Encourage and assist with physical therapy exercises and other therapeutic activities.
• Monitor and report changes in client's condition to the care team.
• Maintain accurate and timely records of client care and services.
• Develop and maintain positive relationships with clients and their families.
