Ascension is seeking a Home Care Support professional for the Live at Home – Chattanooga facility in Chattanooga, TN. This on-site role supports participants with daily tasks and health monitoring on a rotating, full-time schedule.

The position emphasizes compassionate patient care, accurate documentation, and timely reporting of health changes to the supervisor. A high school diploma or GED and a valid driver’s license are preferred and may be required.

Qualifications

• Education: High school diploma or GED preferred.
• Additional Preferences: Valid Driver's License.

Responsibilities

• Assists with daily domestic tasks, including meal prep, light housekeeping, laundry, and errands.
• Maintains accurate care documentation in full compliance with organizational policies and standards.
• Reports participant health changes or concerns promptly to the supervisor.
