Job summary

Nest Home Care LLC is seeking a Home Health Aide to work directly with patients in one-on-one home care plans. You will provide personalized care, maintain a healthy home environment, and document patient progress. Strong communication skills are essential. Prior Home Health or Hospice experience is a plus.

Responsibilities include assisting with daily activities, caring for patient needs, and engaging patients to stay mentally healthy. CPR certification and a valid driver’s license are required.

Qualifications

• Graduated from an accredited Home Health Aid program is required.
• High School Diploma or GED required.
• One-year prior professional experience preferred.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report to supervisor.
