Job summary

Wright Choice Healthcare Agency LLC in Ohio is seeking a Home Health Aide to work with patients in their homes under a one-on-one care plan. You will provide personalized care, build relationships, and maintain accurate records.

The role requires strong communication, attention to detail, and compassion. Prior Home Health or Hospice experience is a plus, and a valid driver’s license and CPR certification are required.

Qualifications

Graduated from an accredited Home Health Aide program.
High School Diploma or GED.
One year prior professional experience.
Driver's license required.
CPR certification required.

Responsibilities

Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor.
Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
Accompany clients to and from appointments.
Engage patients in exercises or other activities
