Adaptive Home Health is recruiting a Home Health Aide (HHA) in Detroit, field-based, to deliver compassionate in-home care. You will provide personal care, assist with mobility, and support daily living activities while documenting care in HCHB.

The role supports our AI-native home health platform and aims to raise the standard of care in the Detroit market. Join a team building a Detroit presence and delivering exceptional patient care with flexible scheduling and strong clinical support.

Qualifications

• Current Michigan HHA certification.
• 1+ years of experience providing personal care in home health, assisted living, nursing home, or hospital setting.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Ability to lift, transfer, and provide hands-on care safely.
• Strong communication skills and professional demeanor.
• Comfortable working independently in patients' homes.
• High school diploma or GED.

Responsibilities

• Provide personal care including bathing, dressing, grooming.
• Assist patients with safe transfers, ambulation, and mobility.
• Support patients with medication reminders as directed by the care plan.
• Monitor and report changes in patient condition to supervising nurse.
• Perform light housekeeping tasks related to patient safety and care.
• Document all care provided accurately and timely in HCHB.
• Communicate clearly with patients, families, and the care team.
• Follow infection control practices and maintain patient dignity and privacy.
• Assist with range of motion exercises and restorative care as ordered.
• Coordinate with supervising RN/LVN for care plan updates and patient needs.
