AVIAN CARE LLC is seeking a Home Health Aide to provide one-on-one care in patients' homes in Minnesota. You’ll follow a personal care plan, build trusting relationships, and maintain accurate care records. Strong communication is needed, and prior home health or hospice experience is a plus.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED.
• One year of professional experience.
• Driver’s license.
• CPR certification.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care and report observations to supervisor.
• Build relationships with patients to keep them mentally healthy and alert.
• Administer medications and immunizations.
• Engage patients in exercises or other activities.
