Job summary

Great Heroes Home Healthcare Agency seeks a compassionate Home Health Aide to provide one-on-one care in patient homes. You will follow personalized care plans, support daily activities, and build trusting relationships with patients and families.

Strong communication, organization, and attention to detail are essential. Prior Home Health or Hospice experience is a plus, with CPR certification required for this role.

Qualifications

• Graduated from an accredited Home Health Aide program.
• One year prior professional experience.
• CPR certification required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Medication reminders
• Engage patients in exercises or other activities
