Job summary

Premier Home Health Providers Inc. is seeking a Home Health Aide to deliver compassionate in‑home care under the supervision of a Registered Nurse. You will assist patients with daily living activities, mobility, meals, and vital signs to support recovery and prevent rehospitalizations.

The role is per diem with a rate of $30–$40 per visit. A CA CHHA certificate and CPR certification are required, along with a valid driver’s license and reliable vehicle.

Qualifications

• Californian Certified Home Health Aide certificate must be held.
• Current CPR certification with AHA is required.
• Valid driver’s license with insured, reliable vehicle is needed.
• At least 18 years of age.
• Ability to read, follow written instructions and document care accurately.
• Able to work independently with minimal supervision.
• Genuine empathy and patience for ill or frail patients.
• Professional, tactful and hygiene‑conscious.

Responsibilities

• Provide personal care including bathing, grooming, oral hygiene and dressing.
• Assist with mobility, exercises and range‑of‑motion per care plan.
• Support meals from preparation to feeding assistance.
• Take and record vital signs as directed.
• Assist with medication self‑administration.
• Keep living space clean, safe and orderly.
• Observe changes and report to supervising nurse.
• Document care provided per organizational standards.
• Perform other duties as required for patient services.
