Job summary

Lifecare Home Health in Naples, FL seeks a Home Health Aide to provide in-home personal healthcare following the patient care plan. You will assist with mobility, feeding, transfers, and daily activities within patients' homes, across age groups.

You will document care, communicate with families and staff, apply infection control, and maintain certifications. Ongoing education and CPR are required to continue employment.

Qualifications

• 1 year of experience as a home health aide (HHA) or certified nursing assistant (CNA) in home health; nursing arts training may substitute per state rules.
• Current CPR certification is required.
• A formal HHA/CNA training certificate that meets state standards is required; ongoing in-service hours may apply.

Responsibilities

• Assist clients with tasks per the Plan of Care (POC).
• Communicate with administrative staff about client needs and condition changes.
• Provide care as outlined in the Home Health Aide Care Plan and document care accurately.
• Contribute to continuity of care and participate in team meetings/conferences.
• Apply infection control and universal precautions in patient care activities.
• Complete required in-service training and maintain competency; meet continuing education requirements.
