Job summary

NuChoice Health Partners is seeking a dedicated Caregiver to provide in-home support for clients in Grove City, PA. You will assist with daily living activities, monitor well-being, ensure safe mobility, and promote independence with compassion and respect.

As part of our Northwest Pennsylvania team, you will collaborate with other caregivers and health professionals, document observations, and communicate clearly with clients’ families to deliver consistent, high-quality care in the home

Responsibilities

• Provide personalized care and support to clients in their homes
• Assist with daily living activities, such as bathing, dressing, and grooming
• Manage medication schedules and administer medications as needed
• Assist with meal preparation and daily living tasks
• Maintain accurate records and communicate effectively with clients and family members
• Work collaboratively with other caregivers and healthcare professionals to ensure high-quality care
