Job summary

Optum is seeking a Direct Care Worker in Appleton, WI to support individuals to live at home by assisting with daily living activities, personal care, and household tasks. The role involves occasional overnight stays, adherence to EVV requirements, and accompanying clients to appointments or errands.

Qualifications

• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire.
• 6+ months of home care experience preferred.

Responsibilities

• Provides or assists clients with activities of daily living including bathing in bed, tub or shower, hair care, teeth/mouth care, nail care, skin care, transfers and ambulation.
• Remains with client overnight for live-in or sleep over when required.
• Uses Electronic Visit Verification (EVV) as applicable and assists with essential household tasks.
• Accompanies client to medical appointments or shopping and completes errands as needed.
• Monitors and records temperatures, pulse, respiration, and blood pressure as ordered within training limits.
• Prepares meals according to client diets and may assist with feeding.
• Maintains a clean, safe home environment with assistive equipment as needed.
