Job summary

Fresh Start-New Beginnings Home Care is seeking a Home Care Aide to work directly with patients in their homes, following a one-on-one care plan and delivering attentive, respectful support. You bring a big heart, reliability, and clear communication to create a safe, comfortable environment for patients.

The role requires detail orientation, organization, and the ability to build genuine relationships. Prior Home Health or Hospice exposure is a plus and strong written and verbal skills are

Qualifications

• Strong verbal and written communication skills required.
• Prior Home Health or Hospice experience is a plus.
• One-on-one care plans in patient homes guided by supervisors.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor.
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
• Give medications and immunizations.
• Engage patients in exercises or other activities.
