Job summary

Home Care Evolution is seeking a compassionate Home Care Aide to provide one-on-one in-home support. You will assist patients with daily activities, follow care plans, and help maintain a healthy home environment with attention to safety and comfort.

Excellent communication, reliability, and a positive attitude are essential. Prior Home Health or Hospice exposure is a plus. A valid driver’s license, HHA program completion, and a high school diploma or GED are required.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED.
• One year of professional home care experience preferred.
• Driver's license required.
• CPR certification preferred.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor.
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
• Give medication reminders.
• Engage patients in exercises or other activities.
