Qualifications

• Must be at least 18 years old.
• Valid driver’s license is required.
• High School Diploma or equivalent required.

Responsibilities

• Assist with health and safety procedures.
• Support people supported in development of skills.
• Assist with community participation and daily activities.
• Provide support to team members and supervisors.
• Assist with person-centered plans and outcomes.
• Assist with medical requests and follow-ups.
• Collect data and report on progress and outcomes.
