Responsibilities

• Provide personalized care and assistance to clients, including bathing, dressing, grooming, and medication management.
• Prepare meals, run errands, and assist with light housekeeping and laundry.
• Escort clients to appointments and social events.
• Monitor and report changes in client condition to the clinical team.
• Maintain accurate records of care and services provided.
• Collaborate with the clinical team to develop and implement individualized care plans.
