Job summary

Trilogy Health Services, LLC in South Bend, IN is seeking a Certified Nursing Assistant (CNA) to provide compassionate daily care based on each resident’s plan. You will assist with hygiene, mobility and monitoring, while documenting changes and communicating with the care team to support comfort and well-being.

Qualifications

• High School Diploma or GED/HSE preferred.
• Active state CNA certification.
• 0–1 years of relevant experience preferred.

Responsibilities

• Help residents with personal hygiene activities, turning, lifting, positioning, and transporting them when needed (activities of daily living)
• Observe and report changes in residents' condition and make independent decisions when needed
• Collaborate with the team to ensure patient comfort and well-being
• Provide companionship, compassion and encouragement to our residents
• Maintain a safe and comfortable environment
