Job summary

Qualifications

• Must be currently certified and in good standing in the state where the facility is located.
• Maintains at least twelve (12) in-service training hours per year as required.
• Complies with resident rights and facility rules at all times.

Responsibilities

• Provide direct resident care under licensed staff supervision.
• Assist residents with activities of daily living (ADL) such as bathing, grooming, and feeding.
• Help with range of motion, positioning, and mobility aids.
• Support resident recreation programs and meals, documenting intake accurately.
• Take vital signs and record measurements as directed.
• Follow resident care plans and report changes to the charge nurse.
• Maintain a clean, safe, and respectful environment for residents.
• Observe and report changes in resident condition including safety concerns.
