Qualifications

• Certification through a state-approved nursing assistant course or enrolled in approved course.
• Must be current on the Wisconsin State Registry within 120 days of hire.

Responsibilities

• Assist with bathing, dressing, grooming and toileting.
• Record vital signs, weights, intake/output per protocol.
• Deliver meals and assist with eating; report chewing/swallowing issues.
• Support restorative nursing including ROM and ambulation with assistive devices.
• Assist with safe transfers using transfer belts or lifts.
• Respond to call lights and assess resident needs.
• Maintain clean, well-groomed appearance and bed linens.
• Report illnesses, changes in condition, accidents or abuse to charge nurse.
• Ensure safe environment; follow facility policies and OSHA guidelines.
• Attend nursing meetings and collaborate with staff; complete required trainings.
