Job summary

Senior Total Life Care, Inc. in Charlotte, NC is seeking a CNA-In ADHC/In Home to provide compassionate patient care in Adult Day Health Center and in-home settings.

The role reports to clinical supervisors and requires assisting with activities of daily living, meals, mobility, and safety per the plan of care. Candidates should have a CNA certification in NC, high school diploma or equivalent, 1–2 years of CNA experience, and a valid driver’s license with reliable transportation.

Qualifications

• High school diploma or equivalent preferred but not required.
• 1–2 years working as a CNA, with at least 1 year experience with frail elderly population.
• Must have NC CNA certification and be listed with NC Nurse Aide Registry.
• Valid driver’s license with personal vehicle insurance; reliable transportation required.
• Able to lift and turn participants and perform daily living activities.

Responsibilities

• ADHC/In Home assists with tasks on a plan of care for participants in the center and/or in the home.
• Provides personal care to participants (bathing, dressing, feeding, toileting, transfers, lifting, ambulation, incontinent care) according to plan.
• Serves meals according to participant diets; prepares simple meals per diet.
• Performs incidental household services essential to participant care (meal prep, housekeeping, laundry, errands).
• Assists with activities in group settings and outings/appointments as needed.
• Obtains vital signs and weights per plan of care and as ordered by physician.
• Monitors oxygen tanks and documents changes; replaces oxygen as needed.
• Reports changes in medical/physical status to the supervisor; performs plan of care procedures under supervision.
• On-call rotation for home care weekdays, evenings, weekends, holidays; aligns with Senior TLC policies.
