StoryPoint Group is seeking caring individuals for Flourish Collection at Rochester to provide personalized care to residents in our community. The role includes assisting with daily living activities, mobility, dining, and medication support while upholding 1440 care standards.

The position offers both full-time and part-time opportunities. Candidates should have a high school diploma or GED, with CNA/MA certifications preferred and prior healthcare or elder care experience valued.

Qualifications

• High School Diploma or GED preferred.
• Experience in healthcare, geriatrics, or with the elderly preferred.
• CNA or MA certification preferred.
• Ability to follow written, oral, or diagram instructions.

Responsibilities

• Uphold 1440 care standards and ensure residents' needs are met.
• Communicate resident changes to Wellness Leader promptly.
• Maintain communications with residents' families.
• Provide compassionate care including bathing, dining assistance, mobility support, toileting, and daily routines.
• Document records and logs; stay flexible with shifts.
• Follow safety procedures and company policies; attend required meetings.
